First-Time Applicants
Please complete Application Form and submit two (2) identical non digitized colour photographs one of which should be certified by the Notary Public who signs Section G (photos are not to be stapled or paper clipped to the form), and an original birth certificate. Where applicable, the following documents should also be sent: A) Original Marriage Certificate B) Original Adoption Register C) Divorce Judgment. For minors, parents/guardians ID (along with letter of guardianship for the latter) should also be sent. Photocopies of documents will NOT be accepted. All Documents submitted with applications will be returned.

Passport Renewals:
Please complete Application Form and submit with your expired passport, two (2) identical color photographs one of which must be certified by the Certifying Official who signs Section F (photos are not to be stapled or paper clipped to the form), and a certified copy of birth certificate. Where applicable, the following documents should also be sent: A) Certified copy of Marriage Certificate B) Certified copy of Death Certificate C) Certified copy of Adoption Register D) Divorce Judgment. For minors, parents/guardians ID (along with letter of guardianship for the latter) should also be sent. Photocopies of documents will NOT be accepted. All Documents submitted with applications will be returned.
Applicants’ signature must remain in box provided. Thumb print box is only for persons unable to sign their names.
Certification: (see section F) can only be done by a Consular Official or a or a Notary Public.
Validity: Adult passports are valid for 10 years, minors for 5 years. Since passport forms are sent to Jamaica for processing, extra care is recommended in completing the form to avoid undue delay.

Personal checks will not be accepted. Money orders or certified checks made payable to the Embassy of Jamaica, or to the Consulate General of Jamaica are to be forwarded directly with the form and other documents.
Replacement of lost or Stolen Passports (see also Renewal of passports above):
In addition to the documents mentioned under the heading
Renewal of Passports, persons who have lost or had their passports stolen must submit a Police Complaint Report and ONE of the following: A) Jamaican driver’s license B) Cancelled Jamaican Passport C) Jamaican voter’s registration card D) Work Permit bearing a photograph from the country of residence E) Identification indicating permanent residency F) Driver’s Licence from country of residence G) Other identification issued by a Federal/State/City authority which has an expiration date (subject to approval by a Consular Officer)
